I don’t want or need client-facing questionnaire. I want a questionnaire ideally so staff members can add a few answers and a document will be prepared. When attempting to use this feature it seems like it’s only designed to be shareable instead of something worked on by the preparer.

In general, you can use questionnaires both client-facing and internally for data entry. Our advanced settings can help you to set up the desired workflow.
Here is use case that describe how you can use Questionnaires for Data Entry and store this information in your Data Management (Reuse Client information) at the same time.
